CDC Home Care Packages

Living an optimal and meaningfully independent life is a goal that every Australian holds.  At Alzheimer’s Queensland we offer a range of services that are customised to support you to remain living at home, one of which is the CDC Home Care Packages.  

 

Alzheimer's Queensland support all ACAT approved individuals including Frail Aged and Dementia.

 

CDC Packages are an Australian Government initiative as part of Home Care Program.  A CDC Package offers care and support services based on the assessed needs of each individual and is delivered based on a mutually agreed service plan.  The term CDC refers to ‘Consumer Directed Care’ which means that the services you receive are based on your choices and that the Package offers you flexibility in what and how to best meet you identified needs.

 

There are currently four levels of CDC Packages, Levels 1-4. Each level of package comprises of a different level of funding that the Government provides.  An ACAT assessment is required to determine the level of package that you are eligible for.  To find out more about eligibility and the steps to access a CDC Package please visit the My Aged Care Website.

Exit Fee

Maximum of $1000.00 chargeable by AQ.

 

The Commonwealth introduced an administrative cost allowance for service providers whereby they are permitted to charge an exit fee.  Please note that this fee defers from provider to provider.  The permissible fee is to cover administrative costs service providers incur in the process of actions such as finalising your services, accounts and transfer of service/care information to another provider.
